The Position
To provide high-quality, evidence-based care and education to patients with diabetes and those requiring weight management support. As part of the Practice team, the nurse will support individuals in managing their condition effectively through personalised care planning, clinical assessments, and lifestyle interventions.
The Responsibilities
Clinical Care
- Conduct comprehensive diabetes and obesity assessments including history, examination, and investigations (e.g., HbA1c, BMI, lipid profiles).
- Support initiation and titration of medications including insulin and GLP-1 receptor agonists (if trained).
- Deliver weight management programmes, including behaviour change, nutritional advice, and physical activity guidance.
- Monitor and evaluate patient progress using clinical outcomes and patient-reported outcomes.
- Work collaboratively with multidisciplinary teams including GPs, dietitians and physiotherapists
Health Promotion and Education
- Deliver structured education sessions (e.g., DESMOND, X-PERT, or in-house programmes).
- Provide 1:1 and group support to patients and carers to promote lifestyle changes.
- Promote preventative strategies to reduce diabetes-related complications and comorbidities.
Care Planning and Coordination
- With the multidisciplinary team, develop individualized care plans with patients, addressing medical, lifestyle, and psychosocial needs.
- Ensure timely follow-ups and referrals to specialist services when necessary.
- Use electronic health records effectively for documentation and communication.
Quality Assurance and Governance
- Participate in clinical audits, service evaluations, and quality improvement initiatives.
- Adhere to clinical guidelines (e.g., NICE, SIGN) and contribute to the development of local protocols.
- Maintain accurate, confidential, and contemporaneous nursing records.
Professional Development
- Maintain NMC registration and undertake regular CPD relevant to diabetes and obesity care.
- Participate in clinical supervision, peer reviews, and professional networking.
- Train and mentor junior staff or student nurses where appropriate.
Experience / Qualifications / Education
Person Specification:
- Criteria
- Qualifications
- Essential: Registered Nurse (NMC)
- Desirable: Postgraduate certificate in Diabetes Care or Obesity
Independent Prescribing qualification
- Experience:
- Essential: 1–2 years in diabetes care or chronic disease
Experience in weight management or lifestyle advice - Desirable: Experience in delivering structured education
Community or primary care nursing background
- Essential: 1–2 years in diabetes care or chronic disease
- Knowledge
- Essential: Good understanding of Type 1 & Type 2 diabetes
Weight management principles and behaviour change - Desirable: Knowledge of insulin pumps, CGMs, and new therapies
Understanding of public health and inequalities
- Essential: Good understanding of Type 1 & Type 2 diabetes
- Skills: Excellent communication and motivational skills
Ability to work autonomously and in a team
Data analysis and audit skills
Teaching and presentation skills - Other Compassionate, organized, and proactive
