B7 Physiotherapist in Rapid Response and Reablement

Guernsey Therapy Group Ltd is looking for an enthusiastic physiotherapist to come and join our experienced and dynamic Rapid Response and Reablement Team. 

The post holder will provide highly specialised physiotherapy, in conjunction with the Rapid Response and Reablement Team patients within their own homes and across various community settings. 

The post holder will be involved in conducting physiotherapy assessments, treatment and physiotherapy diagnosis including identifying and discussing appropriate medical interventions. 

The role of the Rapid/Reablement Team is to reduce delayed discharges, to improve the early supported discharge of patients to their own home, to prevent unnecessary admissions to hospital and to provide increased levels of support for early discharges into the community.  

·                     Degree/ or equivalent in Physiotherapy. 

·                    Evidence of attendance on specialist post-graduate courses relevant to the clinical area. 

·                     Registered Physiotherapist (HPC). 

·                     Member of the Chartered Society of Physiotherapy. 

·                     Documented evidence of continuing professional development. 

·                     Evidence of active participation in in-service training programmes. 

·                     Valid driving licence. 

Experience - Essential

·                     Highly specialised knowledge and experience of a range of approaches to the management of patients requiring rehabilitation 

·                     Previous experience of work in inter- and multidisciplinary teams to include community settings. 

·                     Experience supervising other staff or students. 

Experience - Desirable

·                     Minimum 5 years post-graduation.  

·                     Minimum 2 years post-graduate experience within specialised service area. 

·                     Knowledge or practical use of audit. 

·                     Experience of project work and identifying and implementing evidence-based practice.
